Many of the fatalities were apparent suicides by high school students who attended the same school. On September 21, 2009, the twelfth fatal accident involving Caltrain occurred in San Mateo. The commuter train had just left the San Mateo station around 4:45 p.m. and had not gained full speed when it hit the man. Witnesses have reported that the man, later identified as Peter Manion, was holding two parrots when he was struck by the train. It appeared that Manion froze on the train tracks as Caltrain approached. The trains traveling through the area after the incident were single-tracked and service was delayed for hours after. The cause of the incident is still under investigation.
